How often do you need to replace breast implants?

Breast Implant Replacement

At the time of primary breast augmentation cosmetic surgery, one of the common questions patients have is how long they can expect the results of their surgery to last. In fact, women sometimes hesitate to choose breast enlargement simply because they worry the results may be short-lived and they will have to frequently replace their implants.

A dedicated surgeon will provide clear information on these issues at the time of initial consultation to help a candidate make an informed choice. Board certified female plastic surgeon Dr. Farah Naz Khan provides cosmetic and reconstructive procedures for the breast, body and face to patients in Dallas, Texas and surrounding locations.

Estimated Lifespan of Implants

Although implant manufacturers usually offer a lifetime warranty on breast implants, you should not expect the results of your breast augmentation to last forever. Most women are likely to need breast implant removal or exchange plastic surgery at some stage in the future.

At the same time, it is also a myth that implants would have to be replaced every few years. The fact is that four in five women will be able to enjoy the results of breast augmentation for anywhere between 10 and 20 years, or even more. Only 20 percent of the women may need an implant removal or replacement in their first 10 years after the procedure.

These figures were published by the FDA in 2011. But in the current scenario, you should also take into account that surgical techniques have rapidly improved and new implant qualities are far more durable and resilient than they were a decade ago.

This would mean that the rate of sustainable results with breast implants should be even better now than what the FDA stated in 2011.

When do you need to replace breast implants?

Implant Rupture

In case of a saline implant rupture, you will know about the implant failure within 24 to 48 hours as the saline solution would leak and the implant would deflate like balloon. Although saline is fully biocompatible and causes no harm to your body, you will need to remove or replace the implants for cosmetic reasons.

In case of a silicone implant rupture, it could be a silent rupture. The highly cohesive silicone gel may remain intact in the damaged implant shell or breast capsule, and you may only come to know about it during a routine mammogram or MRI. For your own safety, your surgeon will recommend you to go ahead with an implant removal or exchange.

Capsular Contracture

Sometimes, the scar tissue that surrounds the implant may inexplicably begin to tighten or contract. This could cause breast pain or discomfort, or distort the implant shape or position. In this situation, you will have no other choice but to seek breast implant removal or replacement surgery.

While these are relatively rare medical issues, you should know that a vast majority of women choose breast implant removal or exchange voluntarily for cosmetic reasons, even when there is no medical complication.

Breast Implant Removal After Rupture / Capsular Contracture

Capsular Contracture

Model, not actual patient.

While most women do not consider the possibility that some day they may need to revise the results of their primary breast augmentation, the fact is that more than 40,000 women undergo breast implant removal cosmetic surgery every year. A majority of women choose this procedure voluntarily because of their changed cosmetic preferences, even though they have no medical complications from their implants.

In some cases, breast implant removal or replacement may become a medical necessity if they develop implant rupture, capsular contracture, or another breast implant related complication. Board certified female plastic surgeon Dr. Farah Naz Khan provides cosmetic and reconstructive procedures for the breast, body and face to patients in Dallas, Texas and surrounding communities.

What to Expect during Your Surgery?

Breast implant removal plastic surgery may be performed with or without implant replacement. More women choose implant exchange (removal with replacement) because they want to increase the size of or downsize their current breast appearance.

If breast implant exchange (replacement) is not your choice, chances are that your surgery will be paired with an additional procedure called breast lift. If your implants have survived for many years, they could have caused your breast tissue to stretch.

As a result, your breasts may sag or droop once the implants are removed. This condition can be corrected with a breast lift, which could be performed at the same time with your implant removal.

As a standalone procedure, your breast implant removal surgery may be completed in about one hour. Following general anesthesia, your surgeon will place the incisions preferably in the same location where the original breast augmentation incisions were made. Through the incision, they will remove the scar tissue or capsule that envelopes the implant, and then remove the implant.

In case of saline implants, the removal would be simpler because the implants can be deflated before they are removed.

Silicone Implant Removal with En Bloc Technique

Silicone breast implants are usually removed using a full or partial capsulectomy, where the capsular tissue is fully or partially removed first, and then the implant is taken out. In some cases, the surgeon may decide to perform an en bloc capsulectomy if the conditions are appropriate for it.

In en bloc capsulectomy, the breast capsule is removed with the silicone implant inside remaining intact. In other words, both the capsule and the implant are removed together as one unit. This could be beneficial in a situation where there is a concern about the silicone gel leaking out to other areas following implant damage.

Or, if you are worried about breast implant illness, your surgeon may determine en bloc technique as one of the options at the time of your procedure.

Recovery

Some discomfort and swelling in the incision areas is expected for a few days. Your surgeon will prescribe mild pain medications to keep you pain-free. Within 10 to 14 days, you would be able to return to your routine activities or go back to work.

What is breast lift surgery with implants?

Breast Lift Surgery with Implants

Model, not actual patient.

Breast augmentation with lift, also known as mastopexy implant cosmetic surgery, combines the placement of implants in the breasts with the tightening and lifting of the breast tissue. The procedure is designed to create more comprehensive and natural looking results for women who are bothered by sagging and deflated breasts.

Board certified female plastic surgeon Dr. Farah Naz Khan provides breast lift with implants surgery to patients in Dallas, Texas, and surrounding communities.

Candidacy

If your breast volume has decreased over the years, while the breasts are also drooping with the nipples pointing downwards, you could be a good candidate for breast augmentation with lift plastic surgery. This procedure is also helpful if you wish to correct breast asymmetry.

The purpose of the breast lift is to remove excess skin and tighten and lift the breasts, while the breast augmentation part of the procedure will make the breasts appear fuller, rounder and more volumized.

Procedure

Your surgeon will probably perform breast lift with implants surgery using general anesthesia. The entire procedure can be completed in about 90 to 150 minutes, depending on the extent of tissue correction involved and the surgical technique used.

The incision options for your procedure may include:

  • Circular incision: This is placed around the areola borders in the line between the darker and lighter skin.
  • Lollipop incision: The incision will be placed around the lower half of the areola along with a vertical incision going down to the breast crease.
  • Anchor incision: In addition to the vertical incision, this will include a horizontal incision made across the breast base, just above the natural breast crease.

Length of the incisions will depend on the size of implants to be placed and the degree of breast lift needed. Your surgeon will take care to place the incisions in discreet or concealed locations so that the subsequent scarring is minimized.

Through the incisions, the surgeon will first create an implant pocket to accommodate the implants under the pectoral muscle. Once the implants are adjusted into position, the excess breast skin will be excised and breast tissue will be manipulated to elevate and tighten the breasts. Areolas may have to be resized or repositioned at a higher level to create a complete natural look of your breasts.

Recovery and Results

Your incision areas will have some soreness, bruising and swelling for the first few days. Bruising will resolve by the second week, but the swelling will take a longer time to disappear completely. Mild pain and soreness in the first week can be managed with prescription pain drugs.

You can expect to resume your normal routine somewhere in the second week of your recovery. The results of your breast lift with implants surgery will establish gradually as your breast tissue firms up in its new shape and the swelling resolves. You will be required to use a surgical bra for support in the first few weeks.
